This tragic case involved a mother and her two teenaged children traveling in a van driven by another woman with her two children on the New York State Thruway on their way to a swim meet in Buffalo, New York. The van sustained a "blow out" of its front tire but instead of pulling the van over onto the shoulder of the highway, inexplicably, the woman stopped her van in the right hand moving lane of the Thruway.

An oncoming tractor trailer truck equally inexplicably, did not see the stopped vehicle despite the fact that it was a clear sunny day and there were no obstructions to the driver's vision.
